Amitriptylina Vs Nortriptyline?

I was given nortriptyline to take for treating dizziness and vertigo. After the third time of taking it (5mg at night) I got bad chest pain and aching down my arm, much like you would when having a heart attack. The only other side effect I had was shaking legs. I went back to my doctor and he said he didnt think it could effect your heart but to try amitriptyline instead. I am concerned that this will effect me in the same way as the two medicines have a similar make up. Has anyone else taken these and how did they effect you?

    To my knowledge, at such a low dose of 5 mg/day, nortriptyline should not produce cardiovascular effefcts, especially in non-geriatric patients. Amitriptyline should be in the same category as they are both tricyclics. Sorry I can’t help you out here. Maybe it’s something else, or maybe you are supersensitive to these meds.

    That’s truly odd, but a portion of amitriptyline is metabolized into nortriptyline, so you may want to express these concerns again to your doctor.

    Common side effects of using amitriptyline are dry mouth, extreme weight gain, drowsiness, nervousness, dizziness, blurred vision and insomnia. Some rare side effects include tinnitus, hypotension, mania, psychosis, anticholinergic effects, heart block, arrhythmias, extrapyramidal symptoms, depression, and hepatic toxicity.
    Side effects of Nortriptyline:
    Dry mouth, drowsiness, orthostatic hypotension, urinary retention, constipation, and rapid or irregular heartbeat. Some sexual side effects may be a problem as well. Less commonly, seizures and ECG/EKG changes have been reported, especially in overdose.
    Alcohol may exacerbate some of its side effects and should be avoided.
    However, the incidence of side effects with nortriptyline is somewhat lower than with the first generation tricyclics (e.g. imipramine (TofranilĀ®), amitriptyline (ElavilĀ®)).
